Health Forms and Information

Health Requirement:
All freshmen and new students are required to submit a current health examination form with complete immunization records prior to the first day of school. This exam must be dated on or after August 14, 2023. A parent/guardian must complete the health history section. Any missing component on the exam will delay your student’s registration, and they will not receive their class schedule or be allowed to start school on August 14, 2024.
All students entering 12th grade are required to show proof of having received two doses of MENINGOCOCCAL CONJUGATE VACCINE (Menactra, Menveo, MCV4) prior to entering 12th grade. The first dose is to be administered on or after the students 11th birthday, and the second dose must be administered on or after their 16th birthday. If the first dose is administered when the child is 16 years of age or older, only one dose is required. Your student will not receive their class schedule or be allowed to start school next week until this requirement is met.
Illinois law (Child Health Examination Code, 77 Ill. Adm. Code 665) states all children in the 9th grade shall have a dental examination. The examination must have taken place on or after November 15th, 2023 and a licensed dentist must complete the dental examination form in its entirety.
Athletic Physicals:
If your child participates in sports, a current physical is required for all students participating in the athletic program. This physical will be good for 13 months from the date of the physical.
