    Allergen Information

    Township High School District 214 makes every attempt to identify ingredients that may cause allergic reactions for those with food allergies. Every effort is made to instruct our food production staff on the severity of food allergies.
     
    Please be advised that our menu items may contain allergens or may come in contact with items containing allergens. While we take steps to minimize risk and safely handle foods that contain potential allergens, please be advised that cross-contact may occur, as factors beyond our reasonable control may alter the formulations of the food we serve, or manufacturers may change their formulations without our knowledge. The allergen information provided is accurate and up-to-date to the best of our knowledge. It is based entirely on the information provided by our ingredient manufacturers and suppliers.
     
    Prior to consuming our food, individuals with life-threatening food allergies should confirm the ingredient information on the actual label of the products used. Township High School District 214 will not assume any liability for adverse reactions to foods consumed, or items one may come in contact with while eating at our locations. No allergen information should ever be considered a guarantee, but simply a best-faith effort to serve our customers.
     
    Individuals with food allergies are encouraged to contact their on-site Food Service Director for additional information and support.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
