Applications for a work permit can be picked up in the Main Office.  The application needs to be completed on both sides and returned to the main office with the original Birth Certificate or Passport and original Social Security card.

Reminders about completing this document:

1. The student completes the top portion of the form. 
2. The employer completes the statement section.
3. During the school year, the Statement of Physical Fitness is signed by the School Nurse. During the summer, parents must have the Statement of Physical Fitness signed by their doctor.  
4. The parents sign the application.
5. The Certificate of Employment is filed with the Illinois Department of Labor by the Student Services Office.
