Accident Insurance Forms

High School District 214 has partnered with 1st Agency to provide parent/guardians with the opportunity to purchase low cost student accident insurance for their children while at school, during football/athletics or around-the-clock protection. Communication with 1st Agency Insurance is handled directly with the parent/guardian. Please note 2024-25 insurance can be purchased on or after July 1, 2024.
