Physical Examination and Immunization
A student entering or enrolled in High School District 214 is required to have on file proof of a physical exam and proof of the appropriate immunization series as required by the School Code of Illinois and the Department of Public Health. The physical examination must have been completed within one year prior to the first day of school. Students will be excluded from school if the physical examination and immunization records are not submitted by the first day of school. For more information on these requirements, contact the school nurse.
